Préserver les dépenses sociales des pays pauvres dans le contexte de la pandémie du Covid-19 et de réduction des ressources : tel est l’objectif du moratoire sur la dette. Au Mali, cela a permis, selon la Banque mondiale, de libérer 7,3 milliards de FCFA pour les finances publiques, entre mai et décembre 2020. Dans les couloirs du Centre de riposte anti Covid de l’hôpital du Mali, seule une équipe pluridisciplinaire a le droit de circuler. Les malades, eux, sont confinés dans leurs chambres. Le docteur Garan Dabo, infectiologue et responsable de l’unité, est en première ligne.
« Les patients qui rentrent sont intégralement pris en charge ainsi que la nourriture et d’autres aspects. Cela a un coût et c’est l’État qui assure tout cela. »
Les besoins en médicaments et en matériel sont couverts. La capacité du service est de 96 lits. Plus de 3 000 patients y sont passés en un peu plus d’un an. De nouveaux équipements pour la réanimation ont été achetés et un service de prise en charge des cas suspects a été créé avec le soutien des partenaires extérieurs.
« Ceux qui sont malades, on les confine ici pour qu’ils ne contaminent pas d’autres personnes. Les familles n’ont pas accès », précise le Dr Garan Dabo.
Traditionnellement, et même dans les hôpitaux, ce sont pourtant les proches qui s’occupent de l’hygiène et de nourrir leurs malades. Vingt aide-soignants ont été recrutés avec le concours de Médecins sans frontières.
« Cela règle beaucoup de problèmes parce qu’actuellement, on hospitalise des patients qui sont des cas sévères à critiques. Très souvent, ce sont des patients âgés. En plus des soins médicaux, ils ont besoin d’autres petits soins, donc il faut les accompagner », explique-t-il.
Sur le plan technique, l’unité réussit à faire face aux flux de patients liés à la maladie. Sur le plan social et du point de vue du personnel, les retards de paiement sont récurrents, ce qui mine le moral des agents.
« Ce sont des gens qu’on utilise à plein temps. Ils n’ont pas d’autres sources de revenus. Ils attendent à la fin du mois ce salaire-là pour subvenir à leurs besoins. Par conséquent, ils ne doivent pas attendre cinq à six mois pour être payés », ajoute le Dr Garan Dabo.
Dans le pays, le plan de riposte, en deçà des objectifs initiaux, représentait 64 milliards de FCFA soit 0,6 % du PIB en 2020, une somme supérieure aux économies générées grâce à l’initiative de suspension de la dette du FMI et de la Banque mondiale.
