Alors que la campagne vaccinale risque de se heurter aux réticences des anti-vaccins, c’est pour l’institution, qui prône également la vaccination des enfants et adolescents, le seul moyen d’atteindre l’immunité collective.
« “Obligation” n’est pas un gros mot quand il s’agit de vacciner contre la Covid-19 » : c’est le titre du dernier communiqué de l’Académie nationale de Médecine publié mardi 25 mai. Pour l’institution, il devient en effet indispensable de rendre le vaccin contre le Covid-19 obligatoire pour de nombreuses professions et de vacciner enfants et adolescents, le seul moyen selon elle d’atteindre « une immunité collective suffisante pour contrôler l’épidémie.
Déjà évoquée il y a quelques mois, la question de l’obligation vaccinale se heurtait alors à la pénurie de vaccins et aux incertitudes sur leur niveau d’efficacité, mais « avec un taux d’efficacité de 90 % à 95 % contre les formes graves de Covid-19, les vaccins actuellement homologués en France […] remplissent les conditions qui permettent de recourir à l’obligation vaccinale », juge l’institution.
« Les mesures individuelles (gestes barrières) et collectives (couvre-feu, confinement) sont incapables de contrôler dans la durée » l’épidémie de Covid-19, « redoutable, en particulier socialement », argumente-t-elle.
Des « hésitants » et des « opposants »
Or, « le dynamisme de la campagne va se heurter à l’obstacle des hésitants et des opposés à la vaccination », deux catégories estimées chacune à 15 % de la population, poursuit-elle.
Il sera donc « très difficile d’obtenir avant la fin de l’été un taux de couverture vaccinale qui assurerait une immunité collective suffisante pour contrôler l’épidémie, soit 90 % de la population adulte ou 80 % de la population totale (enfants inclus) », estime l’organe consultatif, qui se prononce sur les questions de santé publique et d’éthique médicale.
Jugeant qu’« une telle obligation ne pourra être instaurée que de façon progressive », elle recommande de rendre tout d’abord « exigible » la vaccination dans toute une série de catégories, à commencer par les professions essentielles (enseignants, professions de santé, police et fonction publique en général, services d’approvisionnement en alimentation, eau, énergie, argent liquide et matériel informatique).
En mars, l’Académie recommandait déjà la vaccination obligatoire pour les soignants.

L’académie estime qu’il faudrait également appliquer cette obligation aux « activités professionnelles comportant un contact avec le public » (commerces, restauration, hôtellerie, établissements culturels et sportifs), « en préalable à toutes les activités comportant déplacements ou rassemblements » (voyages internationaux, cures thermales, événements associatifs), aux étudiants « avant la rentrée universitaire 2021 » et aux donneurs de sang.
Elle ajoute que « la vaccination contre le Sars-CoV-2 aux adolescents et aux enfants devrait être envisagée dès que les protocoles vaccinaux seront homologués dans ces tranches d’âge », en dépit du fait que « la Covid-19 est généralement bénigne dans le jeune âge », car cette extension est « nécessaire pour l’acquisition d’une immunité collective ».
