Des scientifiques ont testé une méthode simple et rapide pour savoir si une personne possède des anticorps contre le coronavirus après la maladie ou le vaccin. Leur bonne idée ? Réinterpréter les tests d’hémagglutination utilisés pour déterminer les groupes sanguins.
Après la deuxième dose de vaccin, est délivré le précieux sésame, l’attestation de vaccination qui est l’un des éléments du pass vaccinal, mis en place à partir du 9 juin. Avec celui-ci, les personnes pleinement vaccinées ou présentant un test PCR négatif datant de moins 48 heures pourront avoir accès à des rassemblements de plus de 1.000 personnes ou voyager sereinement à l’étranger. Mais comment savoir rapidement si une personne possède bel et bien des anticorps anti-coronavirus après une infection naturelle ou la vaccination ?
Détourner le test de détermination du groupe sanguin
Les scientifiques de la John Hopkins University ont adapté un test utilisé pour déterminer les groupes sanguins et le rhésus à la détection d’anticorps anti-SARS-CoV-2. Le principe global est similaire : sur des plaques de carton destinées au test ABO, les scientifiques ont fixé une protéine de fusion qui exprime le receptor binding domain (RBD) de la protéine S du SARS-CoV-2. C’est la petite région du spicule qui reconnait le récepteur ACE2 de la cellule et contre lequel la majorité des anticorps neutralisants sont dirigés. Après avoir solubilisé ces protéines avec une goutte d’eau, il faut rajouter une goutte de sang du patient. Si les anticorps sont présents, un agrégat visible à l’œil nu se forme sur la plaque. Cet agrégat est le résultat de l’agglutination des anticorps anti-coronavirus avec la protéine de fusion qu’ils ont reconnue et les globules rouges. Le résultat, semi-quantitatif, apparaît après quelques minutes et une légère agitation.
Les scientifiques ont testé 200 plasmas de personnes testées positives au Covid-19 et 200 sérums de personnes saines. N’ayant pas des échantillons de sang à leur disposition, ils les ont reconstitués en mélangeant le plasma contenant les anticorps avec des globules rouges du groupe O–. Leurs résultats démontrent que ce test est sensibilité à 87 %, soit à peu près autant que les autres tests sérologiques comme l’Elisa et ceux utilisant la cytométrie en flux. Le test Elisa est plus long à mettre en œuvre, mais il est bien plus spécifique : jusqu’à 100 % de spécificité selon les protocoles contre 95,5 % pour ce test d’agglutination.
Les résultats de cette pré-publication de MedrXiv sont en cours de révision par les pairs. Les auteurs imaginent que ce test pourrait confirmer la présence d’anticorps chez une personne, si elle ne peut pas présenter une attestation ou si seulement une déclaration sur l’honneur est demandée.
